Region Sörmland invites you who turn 40, 50 or 60 to participate in the Sörmland health programme, at no cost. The purpose of the programme is to improve health and prevent cardiovascular diseases and type 2 Diabetes. Many of our common diseases are linked to our lifestyle, such as the tobacco use, level of physical activity and eating habits. Early identification of risk factors is important, because an improved lifestyle or early treatment can significantly reduce the risk of falling ill.

You receive an invitation letter to a health care centre in Sörmland when it's your turn.

How it works:

  1. You receive a letter when it's your turn. The letter includes a scheduled appointment at a health care centre in Sörmland.
  2. Come to your scheduled appointment and provide a blood sample. They will also check your blood pressure, weight, height, and waist circumference. This takes approximately 20 minutes.

    You must not eat anything for at least ten hours before taking the blood sample. That means you should not eat or drink after 10:00 pm the evening before the blood sample. In the hours prior to the blood sample, you should not exert yourself physically and preferably not smoke or use snus.

    It’s okay to take your regular medicines with a bit of water before the appointment.

    If you need an interpreter, contact the healthcare centre from where you received the invitation a few days before the appointment.
  3. Log in to 1177.se and answer questions about your health before your appointment.
  • Go to 1177 e-services (in Swedish)
  • Click on Alla övriga tjänster
  • Choose ”Hälsoenkät Sörmlands hälsoprogram”

If you need assistance with the questions, talk to your health care centre from where you received the invitation.

The health questionnaire is available in both Swedish and English. If you require an interpreter, call the healthcare centre from where you received the invitation a few days before your appointment.

You will receive an invitation to your health dialogue once you have provided your blood samples and answered the questions about your health.

Come to your scheduled health dialogue. The appointment lasts about an hour.

During the health dialogue

You get to meet a health care professional. They will share the results of your blood test and you discuss the questions about your health. Therefore, make sure to answer the questions before your appointment.

You will receive advice on maintaining your health and planning what you can do on your own.

After the health dialogue, you can receive additional support if you want and need it.

An interpreter can be present during the dialogue if necessary.

Reschedule your appointment if you are unable to come

Are you unable to make it to your scheduled appointments? Call the health care centre from where you received the invitation to cancel or reschedule.

If you fail to cancel at least 24 hours before in advance, you will be charged for a missed appointment.

Some health care centres, offer rescheduling of appointments digitally by logging in to 1177.se.
